When someone promises to give someone something but never come back to the thread do I just message an OP to report them?

Depends. If you were legitimately scammed out of something (meaning you PAID for it and never got whatever it was you paid for), then yes. If they were just going to give you something out of the blue, then no. It's not like you were scammed. It sucks and it hurts their reputation, but Ops can't make someone give you something for free. The difference is a trainoff or contest; the owner can't just decide to not do it anymore when they have entries.

Depends on the circumstance and how long you've allowed them to comply. You're always welcome to contact an Op about any situation.
